Initially, the patient's personal data, allergy history, immunization history and preferred time and date of visit may have to be required. Our front desk personnel may have to ask about the patient's chief clinical complaint as well as some pertinent history. Special purpose checklists may have to be filled out in order for the specialist to study and evaluate the case in advance.

 
It may also help if laboratory results, X-ray and CT/MR reports (together with actual films preferred), list of medications and records of previous surgeries are brought to ICOHNS in the first visit. It is recommended that ICOHNS-affiliated HMO patients bring with them official authorization letters and identification cards on all visits to ICOHNS.

 
Patients are advised to list down before the clinic appointment their concerns to speed up the process of the initial interview and to facilitate a clear understanding of the consultation.
